GP down south Mental Health Services
GP down south is funded by the WA Primary Health Alliance to provide the South West Mental Health Portal which is a range of primary mental health services delivered through a stepped model of care.
Our team of experienced Mental Health Professionals will provide assessment, care coordination and the development of a recovery plan in partnership with clients. The stepped care model will provide a range of low intensity evidence-based interventions and psychological therapies through a range of modalities including telehealth, e-health and face to face sessions.
To be eligible to access the service, you must:
- Be diagnosed with, or at risk of developing, a mild to moderate mental illness
- Must be financially disadvantaged (eg. Health Care Card or unemployed) or not have access to alternative care
- Reside in the Great Bunbury or South West regions of Western Australia
- Not be in crisis or require urgent assistance
Referrals can be accepted from your GP or other health professionals.
