General Practitioner Lead

Bunbury Chronic Conditions Care Coordination Team (Hospital to Home)

GP down south is currently seeking expressions of interest from a suitably qualified General Practitioner to join the expanding South West Chronic Conditions Care Coordination team based at Bunbury Hospital.  The position is offered initially as a casual role working 3-4 days  a week to 30 June 2019. GP down south is collaborating with WA Country Health Service to implement an innovative model of care focused on preventing hospital readmission and ensuring successful transition of patients with targeted chronic conditions from hospital to home and to primary care.  The role has been made possible through funding provided by the Australian Government under the PHN Program.

The GP will work as a member of the Chronic Conditions Care Coordination team to provide patient focused medical care/advice that meets best practice guidelines and supports self-management in the primary care setting through the development and implementation of chronic disease care/action plans in consultation with the patient’s hospital and primary care teams.

An integral component of the position is the improvement of systems of care utilising a collegiate approach to improve relationships and promote clear and relevant communication between hospitals and primary care providers particularly in the areas of patient referral and discharge.

This position would suit an experienced GP passionate about improving health outcomes for people with chronic conditions.  The ability to build effective relationships with key stakeholders and work in an evolving model of care is essential.

Experience working in the south west of WA or in a regional /remote region is preferable.

Receptionist / Administration officer

Fixed Term Contract (45 hours per fortnight)

GP down south is a not for profit organisation providing community based health and wellbeing programs and services in the Peel and South West regions of WA.

We require an experienced Receptionist/Administration Officer at our Nidjalla Waangan Mia Health and Wellbeing Centre in Mandurah.

People of Aboriginal or Torres Strait Islander descent are encouraged to apply for the position.

Applicants must have a minimum of Certificate III in Business or equivalent office skills to be considered for the position. Relevant experience in a health setting is preferred, but not essential and previous use of Zedmed would be highly regarded.

The role will focus on providing practical assistance to community members to access health care services and assist the Outreach team to identify barriers which impact access as well as administrative support to the teams.

Receptionist – Peel Youth Medical Service

Part time - Mandurah

GP down south is seeking an experienced and reliable Receptionist to join the team within Peel Youth Medical Service (PYMS) and the Peel Health Hub. An interest and previous experience working with young people will be highly regarded, as will previous experience in providing medical front desk reception and administrative services.

The ideal person will be someone who can work effectively and efficiently within a team of clinical care providers. Reporting to the PYMS Practice Manager, you will need to work independently and be willing to provide a range of support services to ensure the smooth operation of the service.

You will be required to work Monday and Tuesday afternoons and all day Fridays.
